Finally reached the hands - tv out works fine now. Input settings for attached gamepad still cannot be saved, not much trouble, don't mind setup it before playing.

Is tv out without frameskip now? Can input settings be saved? Didn't check yet by myself, would like to play battle city with usb joypad attached, my current version of Picodrive has the questioned problems.
Well, I've noticed that I made previous builds without sdl_mixer at all. Here is new build which uses sdl_mixer. Tried playing it for a while and it seems that loop sound bug has gone. Please check if so for you.
In previous build sound was looping even in first turn? If so, maybe getting rid of Mix_VolumeMusic helped, but MixVolume also influences. Here is another build with Mix_Volume also commented: dowload link.
Regarding sound issue I found this topic. Maybe this is not the case, but I commented code setting fadein and volume to try. Could you please check it? Download link

Build was made upon request. I haven't even tried to run it, have no caanoo with me, sorry.
I think you should buy it. It's pretty cheap here. I have both XPlay and Caanoo. XPlay is certainly more powerful for emulation, though I really like my caanoo.
Run Picodrive through gmenu2X. TV-out works this way but with frameskip: in menu frame renews only every second key press, during play sprites sometimes disappear from the screen (played battle city). Is there any solution to fix this frameskip, maybe modifying source? Native caanoo menu seems...
If compiler shows no errors check where main.o is created. Maybe it is deleted by antivirus software. Try using process monitor to monitor file activity.
Looked sources at openhandhelds for wiz and caanoo. It seems they are much the same. Some differences in handling input ( caanoo uses /dev/input/event1, and wiz - /dev/GPIO ), not sure this could affect performance. Maybe caanoo version is slower due to worse ram timings.
I think its not a bad idea to use dosbox special keys for such tweaking ( +/- cycles, +/- frameskip ) - special keys, - though this will not solve the problem with saving new settings. You may also use internal keymapper to reassign keys and save mapper file.
