It's well known that Picodrive works in TV-OUT mode, but the screen is torn :(

The solution : run it through GINGE. Launch your Caanoo version with Ginge and Picodrive will work.

The problem : sound is weird and the game isn't smooth (games like Sonic), but it was the same on Wiz.

Try to run Picodrive through Gmenu2x ;)

It runs fine ?

Yes, no problems with sound, but as you mentioned game is not smooth (looks like it skipping frames). So try it.
I'm using this gmenu2x launcher, with it you can start gmenu like an application.
